Help people make better health decisions through support, encouragement, and health-related education. Support patients living with chronic diseases through one-on-one appointments, groups, and community-based classes. Provide education on diabetes, prediabetes, cholesterol, blood pressure, glucometer teaching, basic nutrition, asthma, tobacco cessation, family planning, exercise, among other topics. Health educator help patients to set goals that help the patient achieve better health outcomes. Offers community classes including exercise groups, cooking workshops, yoga, diabetes management, and gardening.
