Low-income individuals that have a professional referral to program. Requires a minimum of 25% copayment from other resources which may include other agencies, the applicant, family, friends, etc. Must be at or below 300% of the Federal Poverty Level.

Application can only be requested and submitted by professional representative on behalf of recipient such as:

• Social Worker
• Patient navigator
• Case manager
• Clergy
• Counselors
• Teachers

Applications may be requested on website. Friends of Man pays the vendor directly for approved items, and does not provide funds to applicants. Vendors may not apply. Applications are submitted by email or U. S. mail, not fax.
