Eligibility
Pregnant people, new and breast and chest feeding parents, infants, and children under age 5 who are at or below 185% of the Federal Poverty Level and who have a nutrition risk.
Must live in Washington state.

In-person Appointments: Tu, 10am-5pm.
Telehealth Appointments: M-F, 9am-4:30pm
2nd & 3rd W, 9am-7pm
3rd Sa, 9am-1pm.
