Any individual/family who is unstably housed, unhoused or at-risk of losing housing, lives in emergency shelter, uses day center services, couch surfing, doubled-up with family/friends, or lives in permanent supportive housing within King or South Snohomish counties.

Health Care Access (HCA) supports low income and low-income individuals and families throughout King and Snohomish counties to provide support, education, and advocacy on how to access health services and navigate systems of care. Community Health Workers assist with: Health insurance enrollment Referrals to: health clinics, providers, resources Problem-solving to help families access care Basic health education
