Provides case management services for Camano Island seniors.  Develops case plans for the evaluation, treatment and/or care of individuals who, because of age or illness, are unable to arrange for services on their own behalf; assesses each individual's needs; coordinates the delivery of needed services; ensures that services are obtained in accordance with the case plan.
