Assists individuals with securing essential health and social services for older adults, adults with disabilities and those who care for them, including

Helps eligible adults in applying for COPES, a state government program that pays for personal care and other services. Staff can meet clients in their homes or at specific community locations as well as assist via phone and email. Follows up with callers to make sure the referrals were successful and their needs are being met.
