ADS Clients - Older Adults age 60+ ADS Medicaid Title XIX Case Management Core Clients age 18+ who are referred by ADS case managers.

- Client outreach and engagement
- GAIN-SS screenings
- Treatment plan development and coordination
- Individual counseling
- In-home visits for individualized care
- Information and referral for potential clients who are under the age of 60, must be referred by ADS case managers
