A free, evidence-based program that helps individuals during their hospital discharge to navigate medical and social service systems and establish necessary supports of care. Additional assistance such as Medication Management and referrals to community-based resources are provided.

Funds in home case management/personal care, senior nutrition, Family Caregiver Support, other community services such as Minor Home Repair, Respite, Legal Assistance, Title V Senior Employment, Statewide Health Insurance Benefits Advisors (SHIBA), and provides eligibility screening for the WSDA Dental Outreach program for low-income seniors.
