Provides information, assistance, and initial eligibility screening for services to support seniors, people with disabilities, and their caregivers in assessing their needs, and identifying the most appropriate service provider. Helps facilitate access to social service programs, long term care options, and provides follow-up and advocacy to individuals experiencing barriers to services to ensure needs are met. Also conducts community education and outreach for social service providers and private businesses.
